A strong FNP SOAP note must bridge the gap between detailed patient history and a decisive plan. The Subjective section should capture the chief complaint and HPI with specific modifiers, while the Objective section documents the physical exam and vital signs. The Assessment must reflect the FNP's clinical synthesis, often involving differential diagnoses, and the Plan should clearly outline medications, referrals, and patient education points.
